What Experts Say
Amlodipine besylate has a long elimination half-life of roughly 30 to 50 hours, which is the main reason doctors prescribe Norvasc Tablet 10mg as a once-daily medicine. This steady, gradual action avoids the sharp peaks and dips in blood pressure seen with shorter-acting calcium channel blockers, giving patients more consistent protection through the day and night.
Physicians frequently combine amlodipine with an ARB or ACE inhibitor rather than raising the amlodipine dose alone. This combination targets two different pathways of blood pressure control, often improves results, and can lower the chance of dose-related side effects like ankle swelling that tend to increase at higher amlodipine doses.
Hypertension remains one of the most common chronic conditions seen in general practice and cardiology clinics across Pakistan, with a large share of patients unaware of their diagnosis until a routine checkup. Doctors generally recommend regular blood pressure monitoring alongside medicines like Norvasc Tablet 10mg, since consistent control is what actually reduces long-term cardiovascular risk.

- The tablet should be swallowed whole with a glass of water.

- This tablet should not be stopped suddenly without a doctor's advice.
Drug Interactions
- Simvastatin: Combined use raises simvastatin levels and the risk of muscle-related side effects, so the simvastatin dose is usually limited to 20mg daily.
- CYP3A4 Inhibitors (Clarithromycin, Ketoconazole, Itraconazole): These medicines can raise amlodipine blood levels, increasing the risk of low blood pressure and swelling.
- Other Antihypertensive Medicines: Combining amlodipine with other blood pressure lowering drugs increases the overall blood pressure lowering effect and the risk of hypotension.
- CYP3A4 Inducers (Rifampicin): These medicines can lower amlodipine levels and reduce its effectiveness.
- Tacrolimus: Amlodipine may raise tacrolimus blood levels, so monitoring is advised in patients taking both.
- Grapefruit Juice: Regular consumption in large amounts may modestly raise amlodipine levels, so moderation is generally advised.
Risks and warnings
- Peripheral Edema: Swelling of the ankles and feet is the most common dose-related side effect and tends to be more noticeable at the 10mg dose.
- Low Blood Pressure: Amlodipine can cause hypotension, especially when starting treatment or after a dose increase, leading to dizziness or fainting.
- Severe Liver Impairment: Amlodipine is metabolized by the liver, so patients with severe hepatic impairment need a lower starting dose and closer monitoring.
- Worsening Angina: In rare cases, patients with severe obstructive coronary artery disease may experience increased frequency or severity of angina when starting or increasing the dose.
- Pregnancy and Breastfeeding: Amlodipine should only be used in pregnancy if the potential benefit justifies the risk to the baby, and use during breastfeeding should be discussed with a doctor.
- Aortic Stenosis: Patients with significant aortic stenosis should use amlodipine cautiously, as it may reduce blood pressure further and affect heart function.
- Driving and Operating Machinery: This tablet may cause dizziness or fatigue, so caution is advised until the individual response to the medicine is known.
