What Experts Say
Ofloxacin belongs to the fluoroquinolone class and works by inhibiting bacterial DNA gyrase and topoisomerase IV. This action blocks bacterial DNA replication and repair, leading to bacterial cell death. Because it targets enzymes not found in human cells, it offers selective activity against a wide range of gram-negative and several gram-positive organisms.
Clinicians generally reserve fluoroquinolones like ofloxacin for infections where first-line antibiotics are unsuitable or have failed, since overuse contributes to resistance. Completing the full prescribed course is essential, even after symptoms improve, because stopping early allows surviving bacteria to multiply and develop resistance. Dosage and duration depend on the site and severity of infection.
Ofloxacin carries a class warning related to tendon rupture, peripheral neuropathy, and central nervous system disturbances, so prescribers weigh these risks against its benefits before recommending it. It is generally avoided in pregnancy, children, and people with a seizure history. Renal function and concurrent medications should be reviewed before starting therapy.
