What Experts Say
Ofloxacin belongs to the fluoroquinolone class of antibiotics. It works by blocking two bacterial enzymes, DNA gyrase and topoisomerase IV, which bacteria need to copy their genetic material. Without these enzymes, bacteria cannot multiply and eventually die off, clearing the infection from the body.
The 400mg strength is generally reserved for more severe or complicated infections, such as kidney infections, chronic prostatitis or resistant respiratory infections. Doctors often prefer this dose when lower strength options or standard first line antibiotics have already failed or are unsuitable for the patient.
Fluoroquinolones carry genuine safety concerns including tendon damage, nerve related side effects and heart rhythm changes. Doctors usually limit the treatment course to the shortest effective duration and monitor patients with existing heart, nerve or tendon conditions closely throughout therapy.
