What Experts Say
Physicians generally reserve strontium ranelate for patients who cannot use first line bisphosphonates or who have not responded well to them, since it works through a distinct dual mechanism that stimulates new bone formation while also slowing the breakdown of existing bone tissue.
Before starting therapy, doctors typically screen for uncontrolled high blood pressure, ischemic heart disease, peripheral artery disease and a prior history of blood clots, since these conditions raise the risk of serious cardiovascular events during long term treatment with this medicine.
Regular monitoring during treatment usually includes periodic cardiovascular review and routine kidney function checks, and physicians often reassess the ongoing need for therapy at least once a year, since this medicine is generally continued only when safer bone treatment options remain unsuitable, and calcium intake timing is checked at follow up visits.

Drug Interactions
- Calcium Supplements: Reduce absorption of strontium ranelate when taken together, so doses should be spaced apart.
- Tetracycline Antibiotics: Absorption of both medicines may be reduced if taken at the same time.
- Quinolone Antibiotics: Similar to tetracyclines, strontium can interfere with the absorption of these antibiotics.
- Antacids: Aluminum and magnesium containing antacids can lower absorption of this medicine.
- Anticoagulants: Combined use may add to the risk of clotting complications and needs careful monitoring.
Risks and warnings
- Cardiovascular Risk: May increase the risk of heart attack, so it is generally avoided in patients with known heart disease, uncontrolled high blood pressure or a history of stroke.
- Blood Clot Risk: Carries a risk of venous thromboembolism, including deep vein thrombosis and pulmonary embolism, especially in patients with reduced mobility.
- Serious Skin Reactions: Rare but severe hypersensitivity reactions such as DRESS syndrome have been reported and need immediate medical attention.
- Kidney Impairment: Should be used with caution or avoided in patients with severe kidney disease due to reduced drug clearance.
- Pregnancy and Breastfeeding: Not recommended, since safety data in pregnant or breastfeeding women is insufficient.
- Limited Mobility: Extra caution is needed in bedridden patients or those with restricted movement because of the added clotting risk.
