What Experts Say
Paracetamol, also known as acetaminophen, works by inhibiting prostaglandin synthesis mainly within the central nervous system. This reduces pain signals and resets the body's temperature control center in the hypothalamus. The standard adult tablet dose is 500mg to 1000mg every 4 to 6 hours, not exceeding 4000mg in 24 hours.
Clinical guidelines continue to list paracetamol as a first line option for mild to moderate pain and fever because of its favorable safety profile and minimal gastrointestinal irritation compared to NSAIDs. It does not carry the bleeding risk associated with aspirin, which makes it a preferred choice for a wide range of patients including those with peptic ulcer history.
The most important safety concern with paracetamol is liver toxicity from exceeding the maximum daily dose or combining multiple products that each contain paracetamol. Patients with existing liver disease or regular heavy alcohol use should lower their dose or avoid the medicine unless a doctor has reviewed their case.
