What Experts Say
Enalapril maleate is a prodrug that the liver converts to enalaprilat, the molecule that blocks the angiotensin-converting enzyme. This two-step activation explains why blood pressure lowering builds gradually over the first few hours rather than acting instantly like some other antihypertensives.
Physicians often begin therapy at the 5mg strength in elderly patients, those with reduced kidney function, or anyone already on a diuretic, since these groups carry a higher risk of first-dose hypotension. Serum creatinine and potassium are usually checked one to two weeks after starting or adjusting the dose, because ACE inhibition can transiently reduce filtration and raise potassium, particularly with underlying renal artery narrowing.
Pakistan carries a heavy burden of uncontrolled hypertension and diabetes-related kidney disease, and physicians commonly lean on low-cost generics like enalapril to keep long-term therapy affordable. Consistent daily dosing, paired with periodic renal function monitoring, remains the practical backbone of safe long-term ACE inhibitor use.

Drug Interactions
- Potassium Sparing Diuretics: Combined use raises the risk of dangerously high potassium levels.
- NSAIDs: Regular use of painkillers like ibuprofen can reduce enalapril's blood pressure lowering effect and stress the kidneys.
- Lithium: Enalapril can raise lithium levels in the blood, increasing the risk of toxicity.
- ARBs or Aliskiren: Combining these with enalapril increases the risk of kidney problems and high potassium.
- Diuretics: Starting enalapril with a diuretic already on board can cause a sharp drop in blood pressure.
Risks and warnings
- Pregnancy: Enalapril can cause serious harm to an unborn baby and must be stopped as soon as pregnancy is confirmed.
- Angioedema: Swelling of the face, lips, tongue or throat is a rare but life threatening reaction that requires immediate medical attention.
- Kidney Function: Patients with pre existing kidney disease or renal artery narrowing need close monitoring of creatinine levels.
- High Potassium: Enalapril can raise blood potassium, especially when combined with potassium sparing diuretics or supplements.
- First Dose Hypotension: A sharp drop in blood pressure can occur after the first dose, particularly in dehydrated or elderly patients.
- Dry Cough: A persistent dry cough is a common reason patients switch to an alternative class of medicine.
- Dual Blockade: Combining enalapril with an ARB or aliskiren increases the risk of kidney injury and high potassium.
- Surgery: Inform your doctor before any surgery, as anesthesia can amplify enalapril's blood pressure lowering effect.
