Migraine Problem

My husband is suffering from migraine attacks now a days. He has no stress. Sometimes smokes one cigarette. He is suffering from this pain from last 4 to 5 years. His eyes gets full of tears and gets swelling from severe pain. His brain vein (what u say) of left side gets swell too. He is taking medicine but no recovery. This pain starts once a year. No specific time or season or month. What could be the possible outcomes for this and who should be consult?
