How It Works
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall. PBPs are enzymes responsible for cross-linking the peptidoglycan chains that give the cell wall its structural integrity. By blocking PBP activity, Cefixime prevents new cell wall formation during bacterial replication. Without a stable cell wall, the bacterium undergoes osmotic lysis and dies. This mechanism is bactericidal, meaning Cefixime kills bacteria rather than simply inhibiting their growth.
Spectrum of Activity
Cefixime is most active against gram-negative organisms including Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, Haemophilus influenzae, and Neisseria gonorrhoeae. It has moderate activity against gram-positive organisms such as Streptococcus pneumoniae and Streptococcus pyogenes but is not effective against Staphylococcus aureus, Pseudomonas aeruginosa, or most anaerobes. Sensitivity testing is recommended before use in complicated or recurrent infections.
Clinical Note
The full prescribed course must always be completed even if symptoms resolve before the course ends. Stopping an antibiotic course early is the most common cause of antibiotic resistance and treatment failure. Cefixime should not be used to treat viral infections such as the common cold or influenza. In Pakistan, where antibiotic resistance is rising rapidly, Cefixime should only be prescribed after confirming bacterial infection and, where possible, after sensitivity testing.

How to use Caricef Tablet 400mg?
- Caricef tablet may be taken with or without food, though taking it with food reduces the risk of stomach upset.
- The tablet should be swallowed whole with a full glass of water.
- The full prescribed course must be completed even if symptoms improve before it ends. Stopping early risks treatment failure and antibiotic resistance.
- A doctor should be consulted if symptoms do not begin to improve within 2 to 3 days of starting treatment.
- The standard adult dose is 400mg once daily or 200mg twice daily for 5 to 14 days, depending on the infection type and severity as directed by the doctor.
- The dose should not be changed or the course extended without consulting a doctor.
Drug Interactions
All current medicines, supplements, and herbal products should be disclosed to the prescribing doctor before starting Caricef. Key interactions include:
- Warfarin and anticoagulants: Cefixime may enhance the anticoagulant effect of warfarin, increasing bleeding risk. INR should be monitored closely during concurrent use.
- Probenecid: Reduces the kidney excretion of Cefixime, raising its blood levels and increasing the risk of side effects.
- Oral contraceptives: Cefixime may reduce the effectiveness of hormonal contraceptives by altering gut flora. Additional contraceptive precautions should be taken during treatment.
- Other antibiotics including chloramphenicol, tetracyclines, and macrolides: Concurrent use may reduce the bactericidal effectiveness of Cefixime, as these bacteriostatic agents may compete with its mechanism.
- Antacids and iron supplements: May reduce the absorption of Cefixime if taken together. A gap of at least 2 hours between Caricef and antacids is recommended.
Risks and warnings
- Antibiotic resistance: Caricef should only be used for confirmed bacterial infections. Unnecessary use contributes to rising antibiotic resistance in Pakistan. A doctor must confirm the infection before prescribing.
- Penicillin cross-reactivity: Patients with a known penicillin allergy have approximately a 1 to 2% risk of cross-reactivity with cephalosporins. A full allergy history must be taken before prescribing Caricef.
- Kidney disease: Cefixime is excreted primarily through the kidneys. Patients with renal impairment require dose reduction. Kidney function should be monitored during treatment.
- difficile colitis: All antibiotics including Caricef can disrupt normal gut flora and allow Clostridioides difficile overgrowth, causing severe diarrhea and colitis. Persistent or bloody diarrhea during or after treatment must be investigated immediately.
- Pregnancy: Caricef should only be used during pregnancy when the prescribing doctor determines the benefit clearly outweighs the risk. It should not be used in the first trimester without specialist advice.
- Breastfeeding: Cefixime passes into breast milk in small amounts. A doctor should be consulted before taking Caricef while breastfeeding.
- Seizure history: Cefixime should be used with caution in patients with a history of seizures, as accumulation in renal impairment can lower the seizure threshold.
- Driving and machinery: Caricef may cause dizziness in some patients. Caution is advised when driving or operating heavy machinery if this occurs.

Most patients tolerate Caricef well at recommended doses. Side effects are generally mild and resolve after completing the course.
- Diarrhea or loose stools, the most common side effect of oral cephalosporins.
- Nausea or mild stomach discomfort.
- Abdominal pain or bloating.
- Headache or dizziness.
- Skin rash or itching.
Serious side effects are rare. Medical attention should be sought immediately if any of the following occur:
- Severe or persistent diarrhea with blood or mucus, which may indicate Clostridioides difficile-associated colitis.
- Signs of a severe allergic reaction including facial swelling, difficulty breathing, or anaphylaxis.
- Yellowing of the skin or eyes, which may indicate liver involvement.
- Seizures, particularly in patients with kidney impairment where Cefixime may accumulate.
- Significant reduction in urination or unusual swelling, which may indicate kidney dysfunction.
Storage Conditions
