What Experts Say
Cefepime is a fourth generation cephalosporin antibiotic. It works by binding to penicillin binding proteins on the bacterial cell wall, which blocks cell wall synthesis and leads to bacterial death. Its structure gives it a wider spectrum than earlier cephalosporins, covering both gram negative and gram positive organisms including Pseudomonas aeruginosa.
Clinical guidelines commonly recommend cefepime for hospital acquired pneumonia, complicated urinary tract infections, complicated intra abdominal infections when paired with metronidazole, and empiric treatment of febrile neutropenia. Its stability against many bacterial beta lactamase enzymes makes it a preferred option when resistant organisms are suspected or confirmed.
Dosing must be adjusted in patients with reduced kidney function because cefepime is cleared through the kidneys. Accumulation in renal impairment raises the risk of neurotoxicity, including confusion, encephalopathy and seizures, so clinicians monitor renal function closely and adjust the dose accordingly, particularly in elderly patients.
