What Experts Say
Cefepime is reserved for infections that are serious enough to need injectable treatment, often in a hospital. It is a fourth generation cephalosporin, which means it covers a broader range of bacteria than older members of the family. Choice of antibiotic is often guided by culture results and local resistance patterns, and treatment may be switched to an oral drug once the patient improves.
It works by blocking the building of the bacterial cell wall, so the germs burst and die. Many resistant Gram-negative bacteria that older cephalosporins miss are still sensitive to it. It is given by vial, either into a vein or into a muscle, and the doctor decides the route based on the infection and the patient.
Dose depends on the site of infection and on kidney function, because the drug leaves the body through the kidneys. In kidney impairment, high doses can cause confusion or seizures, so the prescribed dose and timing should be followed closely. Watching for rash, severe diarrhea, confusion or twitching during treatment helps catch problems early, and the care team should be told right away.

Drug Interactions
- Aminoglycosides such as gentamicin: Together they can increase the risk of kidney damage, so kidney function is monitored.
- Loop diuretics such as furosemide: These can add to kidney strain when given with cephalosporins.
- Warfarin: Antibiotics can raise the blood thinning effect, so INR monitoring may be needed.
- Live typhoid vaccine: Antibiotics can reduce how well the oral live vaccine works.
Risks and warnings
- Allergy: Do not use in people allergic to cefepime, other cephalosporins, or with a serious penicillin allergy unless the doctor decides it is safe.
- Kidney impairment: High doses can cause neurotoxicity such as confusion, twitching or seizures, so the dose must be adjusted.
- Antibiotic related diarrhea: Severe or bloody diarrhea during or after treatment can signal C. difficile infection and needs medical care.
- Pregnancy and breastfeeding: Use only when clearly needed and advised by the doctor.
- Resistance: Use only for infections the doctor considers bacterial, to avoid resistance.
