What Experts Say
Loperamide works directly on opioid receptors in the wall of the intestine rather than in the brain. This slows down the movement of the gut, which gives the body more time to absorb water and electrolytes from stool. Because very little of the drug crosses into the bloodstream at normal doses, it controls diarrhea without producing the sedation typically linked to opioid medicines.
This medicine is meant for short term relief and works best when the underlying cause of diarrhea is not a serious infection. It should be avoided when there is fever or blood in the stool, since these can point to an invasive bacterial infection where slowing gut movement may delay the clearance of harmful organisms and worsen the illness.
Taking more than the recommended dose does not speed up recovery and can be dangerous. At high doses the drug can bypass its usual limits and reach the brain and heart, leading to serious rhythm disturbances such as QT prolongation, which is why sticking to the labeled dose matters even though this medicine is available without a prescription.
