What Experts Say
Esomeprazole 40mg belongs to the proton pump inhibitor class of medicines, which switch off the acid pump inside the cells lining the stomach wall. This higher strength is typically reserved for more severe or long standing reflux disease, extensive esophageal damage, or cases where a lower dose failed to control symptoms fully.
At this dose, esomeprazole is also used for short courses in Zollinger Ellison syndrome, a rare condition where a hormone secreting tumor drives excessive acid output, and for patients who need extra ulcer protection during extended use of pain relieving drugs. Healing rates for severe esophagitis are generally higher with the 40mg dose compared with 20mg, though most patients still need four to eight weeks of continuous treatment for full mucosal repair.
Because this strength suppresses acid output more completely, longer courses call for closer monitoring. Periodic checks of magnesium and vitamin B12 are advised beyond eight weeks, and doses should be tapered rather than stopped abruptly to avoid a temporary rebound in acid secretion.
