What Experts Say
Codeine is a weak opioid that the liver partly converts into morphine to relieve pain. Paracetamol works through a separate, non opioid pathway in the brain and spinal cord. Combining the two gives stronger pain relief than either drug alone at standard doses.
Because codeine is an opioid, it carries a genuine risk of tolerance, dependence and withdrawal symptoms if used beyond short term pain relief. Regulatory bodies in many countries now restrict codeine combinations to short courses and require a valid prescription for exactly this reason.
Codeine also depresses breathing at higher doses, a risk that grows when it is combined with alcohol, sedatives or other opioid medicines. Genetic differences in how people metabolize codeine mean some individuals convert it to morphine unusually fast, raising the chance of serious side effects even at standard prescribed doses. This is one reason codeine products are used cautiously in children.
