What Experts Say
Lisinopril belongs to the ACE inhibitor class and works by blocking the conversion of angiotensin I to angiotensin II, a hormone that narrows blood vessels and raises blood pressure. This relaxation of blood vessels reduces the workload on the heart, which is why ACE inhibitors remain a cornerstone of both hypertension and heart failure treatment.
Doctors typically start with a low dose and adjust it based on blood pressure response and kidney function, since ACE inhibitors are cleared through the kidneys and can affect renal blood flow, especially at the start of treatment. A mild rise in creatinine after starting therapy is common and usually not a reason to stop the medicine unless it is significant.
The most recognized class side effect is a persistent dry cough, caused by increased bradykinin levels, which leads some patients to switch to an angiotensin receptor blocker instead. Regular monitoring of blood pressure, kidney function and potassium levels is part of routine follow up during long term therapy.
