Hospitals use it mainly as a short term substitute when a patient cannot take oral medicine, such as around the time of surgery, during acute illness, or in intensive care, matching the usual oral dose so blood levels stay stable through the transition. Because the drug is cleared mostly through the kidneys, doctors adjust the dose in patients with reduced kidney function to avoid drug accumulation.

It is given by trained staff through a controlled intravenous infusion, usually over about 15 minutes, with monitoring for injection site reactions and changes in mood or alertness. It is not intended for use at home, and patients are generally switched back to the oral tablet or solution as soon as they can swallow safely and reliably.
