What Experts Say
Tobramycin is an aminoglycoside antibiotic that binds irreversibly to the 30S ribosomal subunit of susceptible bacteria. This blocks protein synthesis and produces a bactericidal effect against many gram negative organisms, including Pseudomonas aeruginosa, as well as several gram positive species.
Applied topically, the drops reach therapeutic concentrations in the tear film and corneal tissue within minutes while limiting systemic absorption. This local action makes tobramycin drops a preferred first line choice for anterior segment infections such as conjunctivitis and bacterial keratitis.
Clinicians generally recommend completing the full prescribed course even after symptoms ease, since stopping early can allow resistant bacteria to survive. Culture testing is advised if the infection fails to respond within a few days, and prolonged unsupervised use is discouraged due to a small risk of corneal toxicity.
