What Experts Say
Doctors across Pakistan usually prescribe nimesulide for the shortest effective period, often five days or less, since prolonged use raises the risk of liver related side effects without giving extra pain relief. A twice daily schedule after meals is generally preferred to ease stomach irritation.
Physicians recommend baseline liver function checks for patients with existing liver disease or those needing repeated courses, since nimesulide carries a hepatotoxicity warning shared by regulators in many countries. Monitoring matters most in older adults and those already on other liver affecting medicines.
This NSAID is generally avoided in children under twelve and in patients with active stomach ulcers, kidney disease, or heart failure, since these groups face a higher chance of complications. Doctors weigh these risks carefully before choosing nimesulide over safer pain relief alternatives.

- ACE Inhibitors and Diuretics: Nimesulide can reduce the effectiveness of these blood pressure medicines and stress the kidneys further.
- Methotrexate: Nimesulide can raise methotrexate blood levels, increasing the risk of toxicity.
- Lithium: NSAIDs like nimesulide can raise lithium levels in the blood, requiring closer monitoring.
Risks and warnings
- Liver Toxicity: Nimesulide carries a risk of liver injury, so it should not be used for more than five days without medical review.
- Stomach and Gut Irritation: Like other NSAIDs, it can cause stomach pain, ulcers, or bleeding, especially with long term or high dose use.
- Not for Children Under 12: Nimesulide is not recommended for young children due to a higher risk of liver related complications.
- Pregnancy Risk: This medicine should be avoided during pregnancy, particularly in the third trimester, since NSAIDs can affect fetal circulation.
