What Experts Say
Paroxetine works by blocking serotonin reuptake in the brain, gradually raising serotonin levels at nerve synapses. The controlled release formulation delays absorption in the gut, which physicians recommend because it lowers early nausea compared with immediate release tablets while keeping the same total daily dose.
Doctors across Pakistan generally start therapy at a low dose and review response after four to six weeks, since the full antidepressant effect builds slowly. Older patients and those with liver impairment need closer monitoring because paroxetine clearance slows with age and reduced hepatic function.
Physicians recommend tapering the dose gradually rather than stopping suddenly, because paroxetine has a short half life and abrupt withdrawal often triggers dizziness, irritability and flu like symptoms. Combining it with other serotonergic drugs is avoided due to serotonin syndrome risk.
