What Experts Say
Oral rehydration therapy works because glucose and sodium are absorbed together through a shared transporter in the small intestine, and water follows this absorption passively. This is why a balanced glucose to electrolyte ratio, rather than plain water or sugary drinks, is central to effective rehydration.
Clinicians favour lower osmolarity ORS formulations because they reduce stool output and vomiting compared with older, higher osmolarity solutions, while still replacing sodium and potassium losses adequately. Potassium chloride content is particularly relevant in prolonged diarrhoeal illness, where ongoing stool losses can lead to low potassium levels affecting muscle and cardiac function.
Physicians generally advise small, frequent sips rather than large volumes at once, since rapid intake can worsen vomiting in an already irritated gut. Pakistan carries a high burden of diarrhoeal disease in children under five, which is why timely oral rehydration is emphasised as first-line management before intravenous fluids are ever considered necessary.

Drug Interactions

- Potassium-sparing diuretics: Concurrent use increases the risk of elevated blood potassium.
- Corticosteroids: Long-term steroid use can alter sodium and potassium balance alongside ORS intake.
- Diabetes medications: Glucose content may require dose adjustment monitoring in insulin or oral hypoglycaemic users.
Risks and warnings
- Severe dehydration: Sunken eyes, no urine output or lethargy needs urgent medical attention rather than oral fluids alone.
- Kidney disease: Patients with impaired kidney function should use electrolyte solutions only under medical supervision.
- Persistent vomiting: If vomiting prevents any fluid retention, intravenous rehydration may be required instead.
- Diabetes: The glucose content should be considered in patients with blood sugar concerns.
- High fever or bloody stools: These signs need prompt medical evaluation alongside rehydration.
- Infants under 6 months: Use only on a doctor's advice, as fluid needs differ in very young infants.
