What Experts Say
Haloperidol is a first generation butyrophenone antipsychotic. It blocks dopamine D2 receptors in the brain, reducing the transmission that drives hallucinations, delusions and disorganized thinking. This same dopamine blockade explains why it also helps calm acute agitation and mania.
Haloperidol acts fairly quickly after oral dosing, so clinicians favor it for acute psychotic crises and severe behavioral disturbance where rapid control matters. Its high potency means smaller doses achieve strong receptor blockade, but this also raises the chance of extrapyramidal effects such as rigidity, tremor and akathisia, so dose titration and monitoring are essential.
Response varies with age, liver function and other medications, since haloperidol is broken down mainly in the liver. Elderly patients with dementia related psychosis face a documented rise in cardiovascular and infection related mortality with antipsychotics like this one, so specialists reserve its use for confirmed psychiatric conditions rather than general sedation.

Drug Interactions
- CNS Depressants: Combining haloperidol with alcohol, sedatives or opioids increases drowsiness and the risk of breathing problems.
- QT Prolonging Drugs: Certain antibiotics, antiarrhythmics and other antipsychotics taken with haloperidol raise the risk of dangerous heart rhythm changes.
- Levodopa: Haloperidol blocks dopamine receptors and can reduce the effectiveness of levodopa used for Parkinson's disease.
- Lithium: Using lithium together with haloperidol has been linked to increased neurotoxicity, including confusion and severe muscle stiffness.
- CYP3A4 And CYP2D6 Inhibitors: Drugs such as certain antifungals and antidepressants can raise haloperidol blood levels and increase side effects.
- Anticholinergic Drugs: Combined use can worsen dry mouth, constipation and urinary retention.
Risks and warnings
- Increased Mortality In Elderly: Elderly patients with dementia related psychosis face a higher risk of death, mainly from cardiac events or infections, when treated with antipsychotics like haloperidol.
- Movement Disorders: Extrapyramidal symptoms such as tremor, rigidity, restlessness and tardive dyskinesia can occur, sometimes becoming permanent with long term use.
- Heart Rhythm Changes: Haloperidol can prolong the QT interval, raising the risk of serious irregular heartbeat, especially at high doses or with other QT prolonging drugs.
- Neuroleptic Malignant Syndrome: A rare but life threatening reaction marked by high fever, muscle rigidity, confusion and unstable blood pressure requires immediate medical attention.
- Pregnancy And Breastfeeding: Use only if clearly needed, since haloperidol crosses the placenta and passes into breast milk, and newborns may show withdrawal or movement symptoms.
- Parkinsons Disease: Haloperidol can worsen Parkinson's disease symptoms and should generally be avoided in these patients.
