What Experts Say
Betamethasone Valerate is a potent topical corticosteroid. It works by suppressing the release of inflammatory chemicals in the skin, including prostaglandins and cytokines. This reduces redness, swelling, itching and irritation caused by eczema and dermatitis, giving the skin barrier time to repair.
Neomycin Sulphate is an aminoglycoside antibiotic added for activity against common skin bacteria, mainly Staphylococcus aureus and other gram positive organisms. Many inflamed eczema and dermatitis patches carry a bacterial load on the broken skin surface. Adding an antibiotic alongside the steroid helps clear this secondary infection while the steroid reduces the underlying inflammation, addressing both problems together.
Doctors generally reserve this combination for short courses, usually not more than one to two weeks, and only when infection is confirmed or strongly suspected rather than for routine mild eczema. Longer or repeated use raises the risk of skin thinning, stretch marks and neomycin sensitization, and can allow resistant bacteria to develop. Once infection clears, a plain steroid or a simple emollient is preferred for ongoing care.

- Rub in gently until the lotion is absorbed, without excessive pressure on inflamed skin
- Wash your hands after applying, unless the hands are the area being treated
- Avoid contact with the eyes, mouth and open wounds
- Do not cover the treated area with an airtight or occlusive dressing unless a doctor advises it
- Use for the shortest duration advised by your doctor, typically not longer than one to two weeks on the face or two to four weeks on the body
Drug Interactions
- Other Topical Corticosteroids: Using another steroid cream or ointment on the same area at the same time increases total steroid exposure and the risk of skin thinning
- Other Aminoglycoside Antibiotics: Combining with additional neomycin or related antibiotic products raises the risk of allergic skin reactions and, with extensive use, systemic absorption
- Occlusive Moisturizers or Dressings: Applying thick occlusive products or airtight dressings over the lotion increases steroid absorption through the skin and raises the risk of systemic side effects
Risks and warnings
- Skin Thinning: Prolonged or excessive use can cause atrophy, stretch marks and visible thread veins, especially on thin skin areas
- Face and Skin Fold Caution: Avoid extended use on the face, groin, underarms and other thin skin folds where absorption is higher
- Neomycin Contact Sensitization: Prolonged or repeated use raises the risk of developing an allergic reaction to neomycin itself, which can worsen the original rash
- Not for Broken Eardrum or Ear Canal Use: Neomycin can cause ototoxicity if it reaches a perforated eardrum or damaged inner ear tissue
- Not for Primary Infections: Not appropriate as the sole treatment for untreated primary bacterial, fungal or viral skin infections such as chickenpox, cold sores or ringworm
- Caution in Children: Children absorb topical steroids more readily than adults, so use should be limited in both duration and area under medical guidance
- Pregnancy and Breastfeeding: Use only if a doctor considers it necessary, and avoid applying to the breast area before breastfeeding
