What Experts Say
Ointments create an occlusive layer on the skin that traps moisture and increases drug penetration compared with a cream base. For clobetasol propionate, this means Clobeta Ointment reaches deeper into thick, scaly plaques more effectively, which is why dermatologists often prefer it for chronic plaque psoriasis and lichenified skin.
Because absorption is higher under occlusion, doctors are usually stricter about duration and total quantity with ointments than with creams. Continuous use beyond two weeks is discouraged, and the total area treated at one time is kept limited, particularly in children, older adults and areas with thin skin.
Patients should apply sparingly to the affected plaques only and avoid wrapping the area with a bandage or plastic film unless a doctor specifically advises occlusive dressing. Signs to watch for include unusual thinning, easy bruising or stretch marks. Any application near the eyes or on broken skin needs medical guidance first.

Drug Interactions
- Other Topical Corticosteroids: Using another steroid product at the same time increases the combined risk of skin thinning and systemic effects
- Other Topical Medicines On The Same Area: Applying different topical products to the same plaques can alter how much clobetasol penetrates the skin
- Strong CYP3A4 Inhibitors: Drugs like ritonavir or itraconazole may raise systemic steroid levels since the ointment base already increases absorption
- Live Vaccines: Extensive use over large body areas may lower the immune response to live vaccines due to systemic absorption
Risks and warnings
- Higher Absorption Risk: The occlusive ointment base increases drug penetration, so it carries a greater risk of systemic absorption than the cream
- Skin Thinning: Extended use can cause atrophy, striae and easy bruising, particularly on thin skinned areas
- Avoid Occlusive Dressings: Do not bandage or wrap the treated area unless a doctor has advised occlusive therapy
- Limited Duration: Continuous use beyond 2 weeks needs medical review to avoid adrenal suppression
- Infection Risk: Do not apply over untreated bacterial, viral or fungal skin infections
- Pregnancy And Breastfeeding: Use only under medical guidance because absorption is higher with the ointment base
