What Experts Say
Clobetasol propionate ointment carries the same superpotent corticosteroid strength as the cream form but is suspended in a greasier, occlusive base that traps moisture against the skin and improves contact time with the lesion. This occlusive quality enhances drug penetration into thickened, hyperkeratotic plaques far more effectively than a lighter formulation.
Dermatologists favor the ointment form for thick, scaly or lichenified plaques, particularly on the palms, soles and elbows, where a lighter cream often fails to penetrate the hardened, keratin rich skin layer effectively. It is also preferred for dry, cracked skin where added moisture retention supports healing.
Because occlusion increases drug absorption through the skin, treatment on large surface areas or in children is limited to short courses, and application near the face, groin or underarms is generally avoided unless a specialist directs otherwise for a specific indication. Combining the ointment with an additional occlusive dressing further raises this absorption risk and is only done under direct medical guidance.

Drug Interactions
- Other topical steroids: Combining with another corticosteroid product raises the risk of skin thinning and systemic absorption
- Other topical medicines: Applying different topical products to the same area at the same time can reduce absorption or increase irritation
- Live vaccines: Extensive, prolonged use may blunt the immune response to a live vaccine
- Occlusive dressings: Using airtight dressings over the ointment substantially increases systemic absorption and interaction risk
Risks and warnings
- Skin thinning: Prolonged use can cause atrophy, striae and easily visible blood vessels
- Systemic absorption: The occlusive base increases drug penetration, raising the risk of adrenal suppression with extended use
- Face and skin folds: Not recommended for the face, groin or underarms unless a doctor specifically directs it
- Children: Not advised for prolonged use in infants and young children due to higher absorption risk
- Infected skin: Avoid using on skin with an active bacterial, fungal or viral infection unless treated concurrently
- Pregnancy: Use only if clearly needed and under medical supervision, especially over large areas
- Sudden stopping: Abruptly stopping after long term use can trigger a rebound flare
