What Experts Say
Dermatologists classify clobetasol propionate as a superpotent Class I topical corticosteroid. It is generally reserved for skin conditions that do not respond well to milder steroid creams. The ointment base is thick and occlusive, which helps it penetrate deeply into hardened, scaly plaques such as those seen in chronic plaque psoriasis and lichenified eczema.
Clinical guidance limits use to short courses, usually no more than two consecutive weeks, with a weekly ceiling of fifty grams applied across the whole body. Extended or unsupervised use raises the risk of skin thinning, permanent stretch marks and, with large amounts absorbed, suppression of the body's own cortisol production.
Physicians typically avoid prescribing it for the face, groin, underarms or skin folds, since these areas absorb the drug more readily than thick plaque skin. Regular follow up allows a planned treatment break before rebound flares or steroid dependent skin can develop.

How to use Covate Ointment 0.05%?
- Apply a thin layer to the affected skin once or twice daily as prescribed
- Rub in gently until the ointment is fully absorbed
- Wash hands after application unless the hands are being treated
- Do not use for longer than two weeks without consulting a doctor
- Avoid applying to the face, groin or underarms unless a doctor directs otherwise
- Do not cover the treated area with a bandage or dressing unless instructed
Drug Interactions
- Other topical corticosteroids: Combining with another steroid cream on the same area raises the risk of skin thinning and systemic absorption
- CYP3A4 inhibitors like ketoconazole or ritonavir: These can raise systemic absorption of clobetasol when it is used over large or broken skin areas
- Live vaccines: Significant systemic absorption from prolonged, extensive use may reduce the immune response to live vaccines
- Other irritant skin products: Retinoids or salicylic acid on the same area can increase skin breakdown and steroid absorption
Risks and warnings
- Short course only: Do not use for more than two consecutive weeks without medical review
- Weekly limit: Do not exceed fifty grams per week applied across the whole body
- Restricted sites: Avoid the face, groin, underarms and skin folds unless a doctor specifically directs otherwise
- Children: Not recommended under 12 years without specialist supervision due to higher systemic absorption
- Adrenal suppression: Prolonged or extensive use can suppress natural cortisol production and, rarely, cause Cushing syndrome
- Skin thinning: Long term use can cause permanent stretch marks, visible blood vessels and fragile skin
- Avoid occlusive dressings: Do not bandage or tightly cover treated skin unless instructed, since this increases absorption
- Pregnancy and breastfeeding: Use only if clearly needed and under medical supervision, avoiding the breast area while nursing
