Lower Back Pain

My mother feels extreme pain that emerges from her lower back and goes to the feet, getting up and sitting is so tough for her these days, also she has swollen feet and has mild fever almost every day after 6-8 pm. Please, professionals, suggest what the best we can do and also if you can recommend the best neurologist in Karachi who listens to his patients and can treat well. P.S she is diabetic too.
