What Experts Say
Physicians commonly start adult hypertension treatment with Misar Tablet 40mg because it delivers a strong reduction in blood pressure while leaving room to titrate upward if control is insufficient. Telmisartan blocks the AT1 receptor selectively, preventing angiotensin II from constricting blood vessels, and its extended half-life means once daily dosing still covers the early morning surge in blood pressure that raises stroke and heart attack risk.
Doctors generally reassess response after two to four weeks, since the full antihypertensive effect of telmisartan builds gradually rather than appearing within the first few days. For patients who cannot tolerate ACE inhibitors due to a persistent cough, telmisartan and other ARBs are usually the preferred substitute since they do not raise bradykinin levels.
This starting dose also gives physicians a useful baseline for judging whether a patient needs the 80mg strength or would benefit more from adding a second class of antihypertensive instead.

- NSAIDs: Regular use of ibuprofen or diclofenac may reduce the blood pressure lowering effect and worsen kidney function
- Digoxin: Telmisartan can increase digoxin blood concentration, requiring monitoring
- Other antihypertensives: Concurrent use with other blood pressure medicines can cause an additive drop in blood pressure
